If you are attempting to use custom error pages in your Joomla app, and are getting headaches in trying... then welcome to the club. I spent a solid day attempting to do this, and ran into a number of problems with both my host and Joomlas implementation. There are a number of forum threads about this very subject, but none end with a conclusive answer. So this post is merely an attempt to save others from wasting their days on doing the same.
I configured the SEO portion at the same time since it is really easy once you have it installed. Make sure that you have renamed your htaccess.txt file to just .htaccess (you may have to configure your FTP client and/or operating system to view hidden/system files.
In the end the easiest solution was to install SH404SEF. There are a few steps to configuring it, so I'll go through some of those. *I'm using Joomla 1.5*
- Download SH404SEF from here
- Log into your administration panel
- Mouseover 'Extensions' and click 'Install/Uninstall', then browse to where you saved the zip file you downloaded and click 'Upload File & Install' to install SH404sEF. (as follows):

- Mouseover the Components menu, and select sh404SEF.
- You should see the following... you are going to want to click here to turn on the advanced options (you need to do this to enable mod_rewrite).

- The menu should now look like this, and now select sh404SEF Configuration.

- Next click 'Yes' as below to turn sh404SEF on.

- Next select the advanced tab, and select 'with .htaccess'.

- Finally click save in the upper right hand corner of the page.
- Now go back to the sh404SEF configuration page you went to before, and select the '404 Page' tab. Enter whatever information you'd like on your error page here, click save, and when prompt on whether to clear cache, select OK.

- Your custom error page (and sh404SEF for that matter) should be active now. Browse to a URL that does not exist, and text it out.
